Class 044 - Music therapy services; providing wellness services, namely, weight loss programs offered at a wellness center.  Providing information in the fields of health and wellness; healthcare information.  Providing a website featuring information in the field of physical health.

 

Here are your options:

1.) File a statement of use.  If you are using your mark in commerce in connection with your applied-for goods/services, then we should file a statement of use.

You can only choose this option if you are currently  selling or providing the goods / services listed above.

This option costs $200 in attorney fees plus $100 per class in government fees.  You applied for 2 classes, so the cost would be $400.

If you select this option, we will need ALL of the following from A- D listed below:

                                 A) Payment  in the amount of $400 to file the statement of Use.  If you are currently selling your goods/ providing your services.
